xTwo cotton thread tests (a self-prepared cotton thread test (SP-CTT) and the Phenol Red Thread (PRT) test) were performed on 46 dry eye and 37 normal Hong Kong (HK)-Chinese, all subjects being diagnosed by ophthalmologists in a private clinic. The mean SP-CTT value for the dry eye subjects (mean ± SD: 6.2 ± 4.2mm/15s) was significantly lower than that of the normal subjects (8.3±4.3mm/15s). However, no significant difference in the mean PRT value was found between the two groups (dry eye subjects: 10.7±4.5mm/15s; normal subjects; 11.4±4.5mm/l5s).

xThe tear film has three main functions: maintenance of ocular surface health, creation of a smooth optical surface for clear vision, and protection of the eye from foreign bodies and infection. It consists of three main layers [1–4]. The outermost layer of the tear film is the lipid layer. This layer acts to limit evaporation of the aqueous component, as well as providing lubrication for eyelid movement and a smooth optical surface [5]. The middle layer consists principally of water, which contains ions and nutrients to supply the corneal and conjunctival epithlelia, as well as immunological proteins for ocular defence against infections [6].

xThe purpose of this study was to determine the effect of contact lens wear on the tear film and ocular surface of people tolerant or intolerant to contact lens wear. Twenty subjects participated; 11 tolerants and nine intolerants. Their baseline tear film (no lens wear) was analysed with a range of clinical measurements and protein analyses (lactoferrin, sIgA and lysozyme). The tests were then repeated at the end of 6 h of contact lens wear during the day and while lenses were worn. Both tolerants and intolerants showed statistically significant increases in bulbar and overall conjunctival redness after 6 h of lens wear.

xPurpose: Despite advances in materials, discomfort and dryness symptoms are still frequently reported as reasons for dissatisfaction with or discontinuation of soft contact lens wear. Such symptoms are frequently reported in lens wearers and non-wearers in challenging environments. The effect over time of two challenging environments on tear film stability and quantity was tested.

xPurpose: Tear turnover rate (TTR) is defined as the percent decrease of fluorescein concentration in the tears per minute after the instillation of fluorescein. The purpose of this study was to examine differences in TTR in a sample with symptoms of dry eye and an asymptomatic control sample using the Fluorotron Master™ Fluorophotometer. Methods: TTR was measured using the OcuMetrics Fluorotron Master™. It measures the decay of the fluorescence of high molecular weight fluorescein FITC Dextran instilled into the tear film.

xNormal tear film maintains an optically uniform ocular surface, protects the eyes from infections and environmental hazards, washes out cellular debris and foreign bodies and sustains ocular comfort and a healthy corneal epithelium. Mucin, the innermost layer of the tear, changes the hydrophobic corneal surface into hydrophilic. It also makes the corneal surface smooth and helps to balance tear film on the anterior ocular surface. Conjunctival goblet cells produce mucin-MUC5AC and it is supposed to be one of the main gel forming substances in the tears [1].

xContact lens wearers are much more likely to report discomfort than spectacle wearers and non wearers (emmetropes) [1–6], with dryness being the most commonly reported symptom [7]. The ocular symptom “end of day dryness” is most particularly noticed in contact lens wearers [1,3,5] and discomfort due to dryness remains the single largest cause of discontinuation of contact lens wear [8,9]. The underlying causes of contact lens discomfort, however, remain poorly understood, not least due to lack of associations between clinical signs and symptoms of the condition [10–16].

xThe Schirmer test (ST) evaluates the volume of tears in potential dry eye candidates via a strip placed in the lower tear lake. Previously reported inconsistencies in the results may be in part due to the lack of standardization as to the methodology, e.g. the eye position during the test. The purpose of this study was to evaluate the effect of eye position on the ST score.
